Sodium MRI of Core and Penumbra in ischemic stroke patients
نویسندگان
چکیده
Background and Purpose: Diffusionand perfusion-weighted imaging (DWI/PWI) of acute ischemic stroke can be used to identify the ischemic core (DWI) and to estimate the penumbral tissue (PWI-DWI mismatch) which is at risk of infarction but is potentially salvageable. MRI of sodium, a marker of ionic homeostasis that is disrupted after ischemia, within the lesion may be complementary by indicating regions of irreversible injury or by providing an estimate of stroke onset time, the latter of which could be useful for unknown onset stroke patients. However, while sodium changes in the DWI-defined core have been evaluated previously, the relationship of relative sodium signal intensity to penumbral tissue has not yet been investigated. The purpose of this study is to evaluate tissue sodium in the penumbra and core infarct regions from acute to sub-acute times after stroke.

Sodium-23 magnetic resonance imaging has potential for improving penumbra detection but not for estimating stroke onset time.
Tissue sodium concentration increases in irreversibly damaged (core) tissue following ischemic stroke and can potentially help to differentiate the core from the adjacent hypoperfused but viable penumbra. To test this, multinuclear hydrogen-1/sodium-23 magnetic resonance imaging (MRI) was used to measure the changing sodium signal and hydrogen-apparent diffusion coefficient (ADC) in the ischemi...
متن کاملCerebral blood flow threshold of ischemic penumbra and infarct core in acute ischemic stroke: a systematic review.
BACKGROUND AND PURPOSE Cerebral blood flow (CBF) reduction below critical thresholds discriminates between irreversible infarct core, penumbra, and benign oligemia (penumbra that recovers spontaneously). Thresholds are based on animal studies, and their diagnostic accuracy in humans has never been established. The purpose of this study was to assess the evidence available on CBF thresholds for ...
متن کاملViability thresholds of ischemic penumbra of hyperacute stroke defined by perfusion-weighted MRI and apparent diffusion coefficient.
BACKGROUND AND PURPOSE The penumbra of ischemic stroke consists of hypoperfused, but not irreversibly damaged, tissue surrounding the ischemic core. The purpose of this study was to determine viability thresholds in the ischemic penumbra, defined as the perfusion/diffusion mismatch in hyperacute stroke, by the use of diffusion- and perfusion-weighted MRI (DWI and PWI, respectively). METHODS D...
متن کاملBeyond mismatch: evolving paradigms in imaging the ischemic penumbra with multimodal magnetic resonance imaging.
BACKGROUND The ability to quickly and efficiently identify the ischemic penumbra in the acute stroke clinical setting is an important goal for stroke researchers and clinicians. Early and accurate identification of potentially salvageable versus irreversibly infarcted brain tissue may enable selection of the most appropriate candidates for early stroke therapies and identify patients who may st...
متن کامل64-Slice spiral CT perfusion combined with vascular imaging of acute ischemic stroke for assessment of infarct core and penumbra
The aim of this study was to determine the value of computed tomography perfusion (CTP) parameters, including cerebral blood flow (CBF), cerebral blood volume (CBV), mean transit time (MTT) and time-to-peak (TP), in a clinical study of patients with stroke. Additionally, we determined which parameter or combination of parameters are reliable in detecting the presence of an infarct and penumbra....
